PADI Americas recently discovered an error
in some printings of the English language, Imperial EANx32
RDP. No other version of the RDP is affected. Although the
risk of injury is exceedingly remote and no injuries have
been reported, this table is being recalled.
The affected Imperial EANx32 RDP has the
following product number and product code (found in the
lower right corner on the back of the table).

The product code digits indicated by “X” may
vary. The last four digits “PDK7” indicate a recalled table,
irrespective of what the first three digits are.
As a double check, on Table 3, check the 110
foot row under Pressure Group G. The table should indicate a
Residual Nitrogen Time (upper number in white) of 13 and an
Actual Bottom Time (lower number in green) of 12. If this
box is blank, you have a misprinted table.

Problem: Potentially missing
diaphragm cover
SITUATION
IApeks purchased an incorrectly assembled second stage regulator
that was missing
the diaphragm cover. If an Apeks
second stage is missing a diaphragm cover, it could, in
certain situations, allow the diaphragm to become displaced.
A displaced diaphragm could allow water to enter the second
stage case. To date, nobody has been injured as a result of
a missing Apeks diaphragm cover.
Apeks is
asking all consumers to bring their TX, ATX and XTX second
stages,
which have never received prescribed
annual service,
to their nearest retailer
for inspection.

SUBJECT: Recall Notice of Titan/Conshelf DIN
Handwheel Retainer
Aqua Lung is issuing a recall on Titan / Conshelf
DIN Handwheel Retainers (p/n 106056 in America, p/n 124606 elsewhere in the world).
The suspect part, which was made of brass, was used prior to June 2006. The new part, made of stainless steel,
will be stamped “300 BAR MAXI”. The old part made of brass was
stamped “300 BAR MAX” The difference being the letter “I” on the new
part. In 2006, Aqua Lung learned that the part had the potential
of cracking if it were over-torqued by a technician upon
installation. At that time, a Technical Bulletin was issued (reference
Technical Bulletin No. 16 dated March 27, 2006) advising technicians
to change out the brass part for a stronger stainless
steel part when regulators were brought in for service. Since then,
Aqua Lung has concluded that the bulletin has not been effective as
the change outs have not been occurring regularly. In addition, they have now learned that an over-torqued retainer can
break under pressure as well. Because of this, they are formalizing
the change out procedure as a recall. To ensure that all
Titan / Conshelf DIN handwheel retainers are inspected, Aqua Lung has issued this notice to Authorized Dealers and
consumers. The consumer is requested to return their Titan /
Conshelf DIN first stage to an Authorized Dealer where the
DIN handwheel retainer will be inspected and replaced if needed at
no charge to the consumer.

Titan DIN adaptors P/N 103172 with “300 BAR MAX”
stamped on Handwheel Retainer.
Conshelf DIN adaptors P/N 107455 with “300 BAR MAX”
stamped on Handwheel Retainer.

This notice is being issued to pass on important information
regarding all yoke clamp regulators with the serial numbers
between 7010001 and 7053528. We have identified a fault with
a very small number of yoke clamp screws during our assembly
process, which affects the security of the yoke clamp
fitting while under pressure. Although we are fairly
confident that we have caught all faulty items, we believe
there to be no responsible alternative but to institute a
recall to ensure none of these faulty screws have left Apeks.
All customers will need to return the yoke clamp screw to
you for inspection, please see below guidelines for correct
and incorrect thread form. If a faulty yoke clamp screw is
found, then please replace this for a new one and Apeks will
do the same for you.
We will be issuing the recall to the major dive magazines as
well as posting on our website. Apeks apologizes for any
inconvenience this recall might cause, but thank you in
advance for your support in this matter. We are dedicated to

diver safety at all costs; this solution seems the only

Broco
sent out a safety bulletin on
underwater cutting rods #3818UW-25 manufactured between Nov 1,
2006 and Jan 31, 2007.
These were
wrapped with tape too far down the rods, causing a possible
oxygen leak as well as a possible electrical connection problem
resulting in arcing that can damage the collet, collet washer or
torch head.
The minimal
acceptable clearance is .925" from the rod end. Only cutting
rods that are fully seated with visible metal-to-metal contact
with the collet should be used.

Recalls Digital Dive Computers Due to Decompression Hazard
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall
of the following consumer product. Consumers should stop using recalled
products immediately unless otherwise instructed.

product: Oceanic and AERIS Digital Dive Computers
Units:

Manufacturer: Pelagic Pressure Systems, of San Leandro, Calif.
Hazard:
When performing a switch from one gas to another during a dive,
the dive computer's display will lock up and not return to the main
dive screen that displays dive times. This can cause divers to enter
decompression unknowingly or the diver could ascend prematurely, resulting in decompression sickness.
Incidents/Injuries: Pelagic has received a report of two dive computers malfunctioning. No injuries have been reported.
Description and Models: The recall involves Oceanic-brand ATOM 2.0 dive
computers with serial numbers 1 through 2,079 (Revisions 2E, 3A,and 3B)
and AERIS-brand EPIC dive computers with serial numbers 1 through
712 (Revision 1A), which can be accessed and viewed on the computer's display. Also, the
serial number and date of manufacture are
printed on the bottom of the unit (Oceanic ATOM 2.0 from August 23 to
November 23, 2006, and AERIS EPIC from October 18 to November 14, 2006).
This recall does not include any other Oceanic or AERIS brand dive
computers.
Sold at:
Authorized Oceanic dealers sold ATOM 2.0 dive computers nationwide from August 2006 through February 2007. Authorized AERIS dealers
sold EPIC dive computers nationwide from October 2006 through February
2007. Both computers sold for between $670 and $950.

Remedy:
Consumers should stop using the recalled dive computers and take
them to an authorized Oceanic or AERIS dealer to get a free software
upgrade.
